Pediatric malignancies are more common in the Northeast, and boys are at higher risk than girls, study finds

By Serena Gordon
HealthDay Reporter

MONDAY, June 2 (HealthDay News) — A sweeping government study of childhood cancers has found numerous differences in cancer types depending on a child’s age, sex, race and where he or she lives in the United States.

White children had the highest incidence of all cancers, the researchers found, and youngsters in the Northeast were diagnosed with cancer more often than children in other parts of the country.

The study also found that boys were more likely to have a pediatric malignancy than girls, and that adolescents are more likely to have cancer than are younger children.

“We looked at the childhood cancer incidence rate from 2001 through 2003, and further looked at the data by age, sex, ethnicity and U.S. census region,” said the study’s lead author, Dr. Jun Li, an epidemic intelligence office for the U.S. Centers for Disease Control and Prevention (CDC) in Atlanta.

“We identified 36,446 cases of childhood cancer during these three years, which is about 166 per every million,” he said.

Li said the information for the study, which represents more than 90 percent of the U.S. population, came from 39 National Program of Cancer Registries and five Surveillance, Epidemiology and End Results (SEER) databases.

Results of the study are published in the June issue of the journal Pediatrics.

Three cancers accounted for about 60 percent of all childhood cancers. Leukemias were the most common childhood malignancy, affecting just over 26 percent of youngsters with cancer. Central nervous system tumors, such as brain tumors, were the next most common type of pediatric cancer, affecting about 17.6 percent of children with the illness. Lymphomas affected about 14.6 percent of kids with cancer, according to the study.

Overall, boys were more likely to develop cancer than girls. The incidence rate for boys was 174 per million, while the incidence rate for girls was 157 per million. The type of cancer each sex commonly developed also varied. Boys were more likely to have lymphoid leukemia, non-Hodgkin lymphoma, Burkitt lymphoma, hepatoblastoma, osteosarcomas, and more. Girls were more likely to develop kidney cancers, thyroid cancers and malignant melanomas.

The cancer incidence rates for those between 15 and 19 were 210 per million, while the incidence rate in children 14 and under was about 151 per million.

White children were the most likely to have cancer, with an incidence rate of 173 per million. The rate for black children was 118 per million, 131 per million for Asian/Pacific Islanders, and 164 per million for Hispanics. American Indians and Alaska Natives had the lowest rates, with 97 per million.

Geography appeared to make a difference as well. Kids in the Northeastern part of the country are most likely to develop cancer, with an incidence rate of 179 per million. In the Midwest, the rate was 166 per million; in the South, it was 159 per million; and in the West, it was 165 per million. Interestingly, the study also reported that the Northeast, despite having the highest cancer rate, also has the lowest death rate from pediatric cancers.

Li said the researchers weren’t able to identify the reasons for the differences in this study, but he believes the data will lay the groundwork for future research. Knowing these differences may help other scientists target their research, he added.

“This is an interesting study, but as a practicing oncologist, I won’t be advising families any differently. And, as a father of three sons, I wouldn’t have any added concern as a parent living in the Northeast,” said Dr. Adam Levy, a pediatric hematologist and oncologist, and director of pediatric neuro-oncology at the Children’s Hospital at Montefiore in New York City.

“My fear is that people may over-interpret this study, and parents really don’t need added anxiety. We’re still talking about very rare pediatric cancers and mild differences. Mostly, this gives epidemiological researchers clues. Parents don’t need to become overly worried by this,” concluded Levy.

Problems with maintaining an erection may foretell heart trouble ahead for men with type 2 diabetes, two new studies show.

A number of past studies have found a connection between erectile dysfunction (ED) and heart disease. But the new findings, published in the Journal of the American College of Cardiology, confirm that ED often precedes and predicts heart trouble.

This, say researchers, suggests that men with ED should be especially vigilant about controlling heart disease risk factors.

In one study, Italian researchers found that among 291 men with type 2 diabetes, those who also had ED had twice the risk of suffering a heart attack, stroke or other cardiovascular complication over the next four years.

At the start of the study, all of the men had had evidence of “silent” heart disease — meaning they had plaque buildup in their arteries on imaging tests, but no heart disease symptoms, such as chest pain. Having ED seemed to pinpoint those men who were at particular risk of a complication.

There was some good news as well, however: Taking cholesterol-lowering statins appeared to reduce the risks associated with ED, according to the researchers, led by Dr. Carmine Gazzaruso of the Clinical Institute “Beato Matteo” in Vigevano, Italy.

In the second study, Hong Kong researchers found that among diabetic men with no indications of heart disease at the outset, those with ED were 58 percent more likely to die of heart disease, or have a heart attack or other non-fatal cardiac “event.”

“Erectile dysfunction is an important warning sign of future adverse heart events or even death,” study chief Dr. Peter Chun-Yip Tong, of the Chinese University of Hong Kong, told Reuters Health.

The main reason, he explained, is that ED is an early manifestation of the blood vessel damage caused by diabetes and other risk factors for heart disease, such as high blood pressure.

Tong recommended that all men with diabetes tell their doctor if they begin to have problems getting or maintaining an erection. They can then have a comprehensive assessment of their cardiovascular risk factors — such as measurements of their blood pressure, cholesterol, waist size and kidney function — and work on getting those under control.

Indeed, the Italian study suggests that diabetic men with ED can cut some of their heart risks by using a statin. Gazzaruso’s team found that among patients with ED, those who were on a statin had a one-third lower chance of suffering a heart attack or other complication during the study.

There was also evidence that men taking a class of ED drugs called PDE-5 inhibitors, which includes Viagra, had lower heart risks. According to the researchers, this may reflect the fact that the drugs improve the function of the inner lining of artery walls.

The bottom line, according to Tong, is that men with diabetes and ED should have all of their modifiable heart risk factors “identified early and treated aggressively.”

JAMA asserts that the articles “illustrate that clinical trial articles and review articles related to rofecoxib frequently were written by unacknowledged authors who were employees of for-profit information industries, and often attributed first (or primary) authorship to academically affiliated investigators who either had little to do with the study or review or who did not disclose financial support from the company.”

Exposure to pesticides, nerve agents and other chemicals may explain the chronic, multi-symptom health problems experienced by up to one-third of Gulf War veterans, U.S. researchers said on Monday.

They said an analysis of a host of studies offers compelling evidence that the fatigue, muscle or joint pain, memory and sleep problems, rashes and breathing troubles experienced by these veterans are due to chemicals known as acetylcholinesterase inhibitors and organophosphates, which includes nerve gas.

“Convergent evidence now strongly links a class of chemicals — acetylcholinesterase inhibitors — to illness in Gulf War veterans,” Dr. Beatrice Golomb of the University of California, San Diego, said in e-mailed comments.

She said some of the chemicals linked to these illnesses continue to be used in agriculture, and in homes and offices for pest control in the United States and throughout the world.

Golomb’s prior research found that pills known as carbamate pyridostigmine bromide were given to service members to protect against exposure to nerve agents — a practice that has since been discontinued.

For the latest study, Golomb combed through several studies linking Gulf War veterans’ symptoms with all of the chemicals. She found that returning Gulf War veterans who had been exposed to chemicals suffered multi-symptom complaints at a higher rate than those who were not deployed, or who were deployed elsewhere.

“Evidence, taken together, provides a case for causal connection of carbamate, organophosphates and acetylcholinesterase inhibitor exposure to illness in Gulf War Veterans,” Golomb wrote in the Proceedings of the National Academy of Sciences.

She also found a link between the amount of exposure to the chemicals and how common symptoms were in these veterans.

Golomb believes genetic variants make some people more susceptible to such chemicals, and when exposed, these people had a higher risk of illness.

“A lot of attention has gone to psychological factors in illness in Gulf War veterans,” Golomb said. But she said the ground conflict in the Gulf War lasted only four days, unlike the current conflict.

“Psychological stressors are inadequate to account for the excess illness seen,” she said.

She said this knowledge should help protect troops from such problems arising in the future. Her team is also looking at ways to mitigate symptoms in Gulf War veterans.

The study is available online at http://www.pnas.org/cgi/doi/10.1073/pnas.0711986105.

People who drink diet soft drinks don’t lose weight. In fact, they gain weight, a new study shows.

The findings come from eight years of data collected by Sharon P. Fowler, MPH, and colleagues at the University of Texas Health Science Center, San Antonio. Fowler reported the data at the annual meeting of the American Diabetes Association in San Diego.

“What didn’t surprise us was that total soft drink use was linked to overweight and obesity,” Fowler tells WebMD. “What was surprising was when we looked at people only drinking diet soft drinks, their risk of obesity was even higher.”

In fact, when the researchers took a closer look at their data, they found that nearly all the obesity risk from soft drinks came from diet sodas.

“There was a 41% increase in risk of being overweight for every can or bottle of diet soft drink a person consumes each day,” Fowler says.

More Diet Drinks, More Weight Gain

Fowler’s team looked at seven to eight years of data on 1,550 Mexican-American and non-Hispanic white Americans aged 25 to 64. Of the 622 study participants who were of normal weight at the beginning of the study, about a third became overweight or obese.

For regular soft-drink drinkers, the risk of becoming overweight or obese was:

26% for up to 1/2 can each day

30.4% for 1/2 to one can each day

32.8% for 1 to 2 cans each day

47.2% for more than 2 cans each day.

For diet soft-drink drinkers, the risk of becoming overweight or obese was:

36.5% for up to 1/2 can each day

37.5% for 1/2 to one can each day

54.5% for 1 to 2 cans each day

57.1% for more than 2 cans each day.

For each can of diet soft drink consumed each day, a person’s risk of obesity went up 41%.… read more

The dangers of lead in some toys are well-known, but there are plenty of other ways people can be exposed to the metal.

Young children are especially at risk of harm because their bodies are growing quickly. They can suffer damage to the brain and nervous system, slowed growth and other problems.

In adults, excessive lead exposure can lead to problems in reproduction, high blood pressure, memory and concentration problems and other effects.

Levels of lead in the air have plunged since the late 1970s with the removal of lead from gasoline. Today, most lead in the air comes from industrial plants, and it’s a problem chiefly in urban and industrialized areas, the U. S. Environmental Protection Agency says.

Other potential sources:

_Deteriorating lead paint can produce lead dust and chips that children swallow. The federal government banned lead paint from housing in 1978, but older homes may have it.

_Soil can become contaminated and be carried indoors.

_Drinking water can pick up lead from pipes or solder in older homes. Consumers can ask their local health departments or water suppliers about having water tested.

_Traces of lead can be brought home on hands or clothes from jobs that involve working with the metal. The federal government recommends that workers in such jobs shower and change clothes before going home, and wash work clothes separately.

_Food and liquids stored in lead crystal or lead-glazed containers may pick up the metal.

_Some folk remedies contain lead.

_Lead is used in some hobbies, such as making pottery or stained glass, or refinishing furniture.

Rate of Kidney Disease Jumps by 30%
The estimated number of American adults living with chronic kidney disease increases from 20 million to 26 million

An astonishing 26 million American adults are estimated to be living with kidney disease, and most are completely unaware of their condition, according to a new study published today in the Journal of the American Medical Association.

This number increases the most recent estimates of the prevalence of chronic kidney disease (CKD) by 30%, from 10% of the US population (1988-1994) to 13.1% (1999-2004). The data from 1988-1994 was the source of the statistic that 20 million American adults have CKD.

“Our study demonstrates chronic kidney disease in the United States is more common than previously appreciated. However, less than 1 in 10 individuals with kidney disease is aware they have a problem,” said Josef Coresh, MD, PhD, lead author of the study and professor in the Departments of Epidemiology and Medicine at Johns Hopkins University. “The medical community is starting to realize that chronic kidney disease is a serious concern, similar to the increased awareness of hypertension in the 1970s and diabetes in the 1990s.”

“We at the National Kidney Foundation interpret this increase in the rate of CKD as a call to action for doctors, people most at risk, and their families,” says Allan J. Collins, MD, FACP, President, National Kidney Foundation.

“The low level of awareness of CKD is challenging the National Kidney Foundation to reach out to individuals with diabetes and high blood pressure, who are at increased risk,” says Joseph A. Vassalotti, MD, Chief Medical Officer, National Kidney Foundation

In people with CKD, the kidneys are less able to perform vital functions that help maintain overall health, including filtering wastes and excess fluids from the blood. People at increased risk include those with diabetes, high blood pressure, cardiovascular disease and a family history of kidney disease. Minority groups and older adults are also at increased risk.

Estimates of the rate of CKD are based on data collected as part of the National Health and Nutrition Examination Surveys (NHANES), which measure health risks in a representative sample of non-institutionalized American adults.

Between 1994 and 2004, the rate of Americans with mild, moderate and severe CKD all increased. The study could not estimate the number of individuals with CKD in stage 5 – the most severe form of the disease, kidney failure, in which people need dialysis or transplant to survive. Also concerning, the authors found that the majority of people with CKD didn’t realize they had it.

“Through the Kidney Early Evaluation Program (KEEP), which provides free kidney screening for those at increased risk, the foundation plans to test 4,000 American adults each month. Early detection of CKD allows more time for evaluation and intervention,” says Vassalotti.

“Americans and their physicians should be aware that chronic kidney disease is common, has treatable components, and its progression can be slowed substantially. Blood pressure control with agents that protect the kidney, blood sugar control, and avoiding medications toxic to the kidneys are the most important factors for patients with kidney disease to be educated about,” said study co-author Andrew S. Levey, MD, Chief of Nephrology at Tufts-New England Medical Center and Work Group Chair of the NKF’s KDOQI Clinical Practice Guidelines for Chronic Kidney Disease: Evaluation, Classification, and Stratification.

Some facts about CKD:

CKD can increase the risk of cardiovascular disease and other complications. Indeed, the most common cause of death among people with CKD is cardiovascular disease.

In the U.S., the two leading causes of kidney failure– which requires regular dialysis or transplantation to sustain life– are diabetes and high blood pressure. When these two diseases are controlled by treatment, the associated chronic kidney disease can often be prevented or slowed down. Healthy lifestyle changes, such as losing excess weight and regular exercise, often help to control — and may even help to prevent — high blood pressure and complications from diabetes.

Kidney disease may be silent in the early stages. It’s important for doctors to include blood and urine tests that check the kidneys as part of regular medical checkups. Early detection and treatment may help to prevent chronic kidney disease from worsening.

——–

Artificial sweeteners are marketed with the promise of weight control, and the vast majority of people who consume them do so to either lose or avoid gaining weight. Folks, this is a fraud of gigantic proportions.

From 1960 to l976, there was virtually no change in the number of Americans who were overweight: roughly 24 percent of the population. However, from the mid l980s to the present, this number has more than doubled to 54 percent! This coincides with the massive infusion of noncaloric chemical sweeteners and sugar-free “diet” foods that are eaten by close to three-quarters of the adult population.

Although several factors contribute to these alarming statistics, I am convinced that our blind acceptance of the most popular of these artificial sweeteners, aspartame (NutraSweet, Equal, Spoonful), plays a significant role in our current weight problems. Far from helping us lose weight, aspartame has been proven to increase appetite, especially cravings for sweets. Imagine “diet” products that help you pack on extra pounds! And aspartame’s downside doesn’t end with weight gain: This sweetener is associated with multiple health problems.

ASPARTAME MAY CAUSE A VARIETY OF DISEASES
Since aspartame came on the market in l981, it has accounted for more than 75 percent of the complaints reported in the FDA’s Adverse Reaction Monitoring system. The most common adverse reactions attributed to aspartame are headaches, dizziness, attention difficulties, memory loss, slurred speech and vision problems. This cluster of symptoms has become so common that it is actually referred to as “aspartame disease”.

Even more serious disorders have a suspected link with aspartame. Is it an accident that the incidence of brain tumors has increased by 10% since l975? John W. Olney, MD, of the Washington University Medical School in St. Louis believes there may be a link between the two. In an article published in The Journal of Neuropathology and Experimental Neurology, he notes that animal studies reveal high levels of brain tumors in aspartame-fed rats. According to Dr. Olney, recent findings show that aspartame has mutagenic (cancer-causing) potential, and the sharp rise in malignant brain tumors coincides with the increased use of aspartame.

Could serious seizures and vision loss somehow be associated with the sweetener? The U.S. Navy and Air Force published articles in Navy Physiology and Flying Safety with this warning: “several researchers have found aspartame can increase the frequency of seizures, or lower the stimulation necessary to induce them. This means a pilot who drinks diet sodas is more susceptible to flicker vertigo, or to flicker-induced epileptic activity. It also means that all pilots are potential victims of sudden memory loss, dizziness during instrument flight, and gradual loss of vision.”

What about multiple sclerosis, chronic fatigue, rheumatoid arthritis, depression and other mood disorders? I have reviewed scores of documented cases of patients with symptoms so severe that they were mistakenly diagnosed with one of these conditions, only to have all signs of disease completely vanish after getting off aspartame.

THE FDA IGNORES SAFETY CONCERNS
Yet the FDA has chosen to turn a deaf ear to repeated requests by scientists, physicians, and consumers to review aspartame’s safety.

Aspartame has spelled trouble from the get-go. The unique property of this chemical, which is 200 times sweeter than sugar, was accidently discovered in l965 by a chemist trying to develop an ulcer drug. Although the FDA rescinded its initial approval because of studies showing that it caused seizures and brain tumors in lab animals, the agency eventually capitulated to political and monetary pressure and in l981 gave aspartame the stamp of approval. In doing so, this bureaucracy overrode the 3-0 decision of a Public Board of Inquiry, which had reviewed the scientific data and had recommended delaying approval pending further studies on the sweetener’s link with brain cancer.

In the intervening years, safety concerns have mushroomed. Ralph G. Walton, MD, Professor of Psychiatry at Northeastern Ohio Universities College of Medicine, reviewed all the studies on aspartame and found 166 with relevance for human safety. Every one of the 74 studies funded by the aspartame industry gave it a clean bill of health, while 92 percent of those independently funded revealed safety problems.

ASPARTAME CAN UPSET BRAIN CHEMISTRY
Once you understand a bit about the chemistry of aspartame, you’ll see why it can cause so many problems.

Aspartame is comprised of two amino acids, aspartic acid and phenylalanine. Aspartic acid acts as an “excitatory” neurotransmitter, or chemical messenger, in the brain, stimulating neurons to fire. Problems can arise when aspartic acid is out of balance with “inhibitory” amino acids that calm things down. Phenylalanine also easily enters the brain, where it is transformed into neurotransmitters that can further interfere with normal brain function.

This is a likely reason why aspartame lowers the threshold for seizures, mood disorders, and other nervous system problems. This altered brain chemistry may also be responsible for the addictive nature of aspartame. Some patients report that getting off diet soda takes more willpower than giving up cigarettes!

A LITTLE MOONSHINE FOR YOU?
The remaining component, which makes up 10 percent of aspartame, may be the most dangerous part. It is a methyl ester that breaks down after ingestion into methanol, a nervous system toxin also known as free methyl alcohol or wood alcohol. Methanol is extremely harmful to the optic nerve. A main ingredient in “moonshine” it was notorious during Prohibition for causing blindness. Methanol is rapidly released into the bloodstream, where it is further metabolized into other harmful components, including formaldehyde (a known neurotoxin and carcinogen) and formic acid (the poison in ant stings).

Is it any wonder that many of the symptoms of “aspartame disease” are neurological and visual? Drinking a diet soda or two (and I’ve had patients who drink at least a liter a day) delivers a powerful chemical rush with decidedly negative effects. With this kind of questionable history, who would want to consume this artificial chemical, particularly when there are natural and healthy sweeteners available?

Restless legs has become one of those diseases that almost seems to have been created during 30 second television commercials. Within the last several years, both doctors and patients alike started to learn about the symptoms of restless legs and treatment thereof. Patients were told to ask their doctor if Mirapex or Requip were right for them… and they did.

The symptoms of restless legs syndrome are simple. The person has an uncontrollable desire to move their feet, usually while trying to sleep. In addition, spouses will also complain that the person affected is “kicking their feet all night long”. Most people with RLS will also have daytime fatigue. If you have any of these symptoms, consult with your physician. There have been many studies that show a good night sleep is essential for health and longevity, this study is evidence that failure to sleep well may take years off your life.

Restless Legs Syndrome Doubles Risk Of Stroke And Heart Disease, Study Shows

ScienceDaily (2008-01-01) — People with restless legs syndrome are twice as likely to have a stroke or heart disease compared to people without RLS, and the risk is greatest in those with the most frequent and severe symptoms, according to new research. … > read full article
